6.10 Conclusions

The wide range of CW agents and toxic industrial chemicals produces patterns of toxic trauma which can be analysed according to actions on somatic systems. In the case of agents which act upon the central and peripheral nervous systems, there is a good understanding of the pathophysiological mechanisms which has led to a systematic approach to treatment with both life support measures and antidotes. The same is true of chemical asphyxiants such as where the understanding of the mechanisms behind relatively common emergency presentations such as carbon monoxide and cyanide poisoning has produced a range of treatment options. The pathophysiology of lung-damaging agents has been studied intensively in the form of acute lung injury and acute respiratory distress syndrome from both physical and toxic causes. However, while detailed protocols for management have been produced in the case of ARDS, these have largely been derived from ARDS from other causes than toxic exposure. Much of the research has been done on animals and the application of mechanisms so derived to man must be done with caution.
